The #1 Factor in Brain Health: Why Blood Flow Matters More Than You Think

Most longevity conversations skip right past the one thing that actually predicts how sharp your brain stays. It's not a supplement. It's not a brain game. It's blood flow. 

If you've spent any time around the longevity conversation, you've heard plenty about supplements, brain games, and whatever the newest biohack is. Almost nobody starts with blood flow. The Amen Clinic, which has done more brain scans than almost anyone, lists it as the first of eleven risk factors in their brain health assessment. Your brain is about 2% of your body weight and uses roughly 20% of your oxygen supply. Nothing about that ratio works without blood getting there efficiently. 

So let's start there: what damages blood flow, what improves it, and a few specific things you can do about it today. 

What Damages Blood Flow to the Brain?

A handful of everyday habits do most of the damage, and most of them are things you already half-know to watch for: 

  • Smoking is the biggest one, by far, though far fewer people smoke today than a generation ago. 

  • Too much caffeine. Caffeine constricts blood vessels, and more than about two cups a day starts to add up. That's not the same as saying caffeine is bad. Your blood vessels constrict and dilate all day long in response to different things, and that's normal. Cold plunging does the same thing temporarily, constricting blood vessels for a couple of minutes, then dilating when you warm back up. The problem isn't constriction itself. It's too much of it, too often, without the dilation side ever balancing it out. 

  • Frequent alcohol use, especially daily, damages the vascular system over time and reduces blood flow. 

  • Dehydration matters more than most people realize. Blood pressure is determined by exactly two things: the diameter and stiffness of your blood vessels, and the viscosity of your blood. If you're dehydrated, your blood is thicker and harder to move, even if your vascular health is otherwise fine. 

  • Sitting. Being sedentary has been compared to smoking in terms of how much it damages circulation, and that comparison holds up even if you already work out. If you get in an hour at the gym and then sit for the next eight to ten hours, the workout doesn't undo what the sitting did. The sitting is what's hurting your blood flow, separately from whether you exercised that day. 

  • Poor sleep rounds out the list, and it's worth taking seriously. Restless sleep, trouble falling asleep, and not getting enough deep sleep all affect blood flow and brain health. Sleep apnea deserves special attention here. It's far more common than most people think, and it's easy to miss. A 2025 analysis by Sönmez and colleagues estimates that roughly 32% of U.S. adults, 39% of men and 26% of women, have obstructive sleep apnea, and it remains largely undiagnosed.[1] If you snore, wake up gasping for air, or never feel rested no matter how many hours you sleep, it's worth getting checked. 

How Do You Improve Blood Flow to the Brain? 

Most of this comes down to vasodilation, the expansion of your blood vessels, which lowers blood pressure and lets more blood reach your tissues, including your brain. There are two specific ways to trigger it that don't get talked about nearly as much as diet and exercise. 

The first is heat. A heating pad on the back of your neck vasodilates the blood vessels running to and from your brain, and it only takes a few minutes. If you've been sitting for a while, that's a fast way to get blood moving again. Saunas work too, more dramatically. A 2017 study by Laukkanen and colleagues, published in Age and Ageing, followed 2,315 middle-aged Finnish men for a median of 20.7 years and found that sauna use 4 to 7 times a week was associated with a 65% lower risk of Alzheimer's disease compared to once-a-week use.[2] Worth noting before you build a routine around it: the study followed only middle-aged men, so we don't yet know whether the same effect holds for women. If a sauna isn't accessible, a warm bath or even a warm drink helps, though the heating pad on the neck is the most targeted, lowest-effort option of the group. 

The second is nitric oxide, a gas your body releases that relaxes and widens blood vessels. It lowers blood pressure, improves the elasticity of your blood vessels as you age (countering the arterial stiffening that drives a lot of the blood pressure increase we see with age), acts as a neurotransmitter that helps brain cells communicate, and helps maintain the health of the endothelial walls lining your blood vessels. 

You release nitric oxide through exercise, nutrition, and sleep, but compression triggers a fast, specific release. That's part of why tools like Normatec recovery boots work, and it's the same mechanism behind Blood Flow Restriction (BFR) training. You don't even need to be exercising

to get the benefit: two to three minutes of compression, even sitting at your desk, triggers that release. If you have a sedentary job where it's hard to get up often, that's worth building into your day. 

Does Movement Actually Help, and Does It Have to Be a Workout? 

Movement and exercise aren't the same thing, and that distinction matters here. Exercise is structured: building muscle, building aerobic capacity. Movement is everything else, and you need a lot of it. 

Here's something that surprises people: standing instead of sitting isn't much of an upgrade for blood flow. Standing still for long periods can actually be worse for circulation in your legs, since it raises the risk of blood pooling, called venous insufficiency. What actually helps is walking, even slowly. A walking pad under your desk, the slow kind, not a treadmill you'd run on, keeps blood moving and supports venous return in a way that standing alone doesn't. They're available for well under $200. 

Aerobic exercise, brisk walking, biking, that kind of thing, supports blood flow, but you need more volume of it for the same benefit. High-intensity training is more efficient for blood flow but can be intimidating or simply not accessible if you're managing an injury or just don't want to push that hard. This is part of why BFR has become such a core piece of how I train: it mimics the effect of high-intensity training, including the nitric oxide release and the vasodilation, without requiring that same level of exertion. Emerging research also links BFR to a release of brain-derived neurotrophic factor (BDNF), likely driven by the same metabolic stress that fuels muscle growth.[3] BDNF plays a role in neural growth and plasticity, helps form new synaptic connections, and supports the health of the same blood vessel lining that nitric oxide protects. 

What's the Bottom Line? 

Brain health doesn't start with your brain. It starts with your blood vessels. The old assumption was that brain cells simply got less healthy as we aged. The more current understanding flips that: it's the blood vessels that age first, and the brain cells suffer downstream. Keep the vessels healthy and elastic, through movement, heat, compression, hydration, and sleep, and you're protecting the actual mechanism that keeps your brain supplied. 

None of it is a new project to manage: 

• A heating pad on your neck for a few minutes 

• Standing up once an hour if a walking pad isn't realistic 

• A few minutes of compression at your desk 

A handful of small habits aimed at one thing that actually moves the needle.
